Cape Regional Physicians Associates Chosen to Participate in National Model to Improve Primary Care, Patient Health

CAPE MAY COURT HOUSE – Five primary care practices from Cape Regional Physicians Associates, an affiliate of Cape Regional Health System, have been selected to participate in the Centers for Medicare & Medicaid Services (CMS) Comprehensive Primary Care (CPC+) Alternative Payment Model (APM) for 2017.
CPC+ is one of four APMs designed by CMS and the Innovation Center that aim to achieve better care for patients, better health for communities, and lower costs through improvement to the health care system. The five-year model benefits patients by:
- helping primary care practices support patients with serious or chronic diseases to achieve their health goals
- giving patients 24-hour access to care and health information
- delivering preventive care; engaging patients and their families in their own care
- and working together with hospitals and other clinicians, including specialists, to provide better-coordinated care
CPC+ also features a redesigned payment structure for Medicare reimbursements. The new model enables CMS to give practices the additional financial resources and flexibility they need to make investments that will improve quality of care and reduce the number of unnecessary services their patients receive.

Beginning in January 2017, CPC+ supports 2,983 primary care practices, comprising 13,090 clinicians and serving more than 1.76 million Medicare beneficiaries. Practices of all sizes and structures are located in each of the 14 CPC+ regions across the country.
“Strengthening primary care is critical to improving healthcare for our community and a smarter way to spend health care dollars,” continued Carrocino. “Through this program, we can support primary care doctors so that they can spend more time with patients, serve patients’ needs outside of the office visit, and better coordinate care with specialists.”
